12/27/2023 EnchantaVaughn Approved QMHPA greeted Ms. Cosby upon arrival to the home setting. Upon arrival to the home setting, Ms. Cosby appeared to be in a positive mood evident by her facial expressions such as a smile on the face. QMHPA engaged Ms. Cosby in a discussion to determine how her morning was. Ms. Cosby discussed with QMHPA that her morning was going well. QMHPA informed Ms. Cosby that this session was going to continue to discuss problem solving skills due to the discussions that have been discussed between QMHPA and her this week. Upon arrival, she did not present any symptoms of S/I. QMHPA began the session by inquiring about Ms. Cosby current thoughts, feelings and emotions. QMHPA encouraged Ms. Cosby to discuss with her symptoms of depression and anger. QMHPA taught Ms. Cosby a skills building activity to assist her with the use the skills taught to explore effective communications that is clear and concise. QMHPA used modeling and instructions to assist Ms. Cosby. QMHPA educated Ms. Cosby on how this skill will assist her with increasing her communications skills to meet her needs, develop friends and increase her social skills. QMHPA also informed her that this skill could also assist her with being able to build that bond with her daughter and others that causes to her to become depressed and experience anxiety on a daily basis. QMHPA encouraged using calming techniques such as deep breathing or stepping away from moment to relax and calm down. Ms. Cosby appeared to be receptive to QMHPA’s interventions and services that were provided to her during the session. Ms. Cosby shared that she made several attempts to journal last night but couldn’t. Ms. Cosby stated, “I can stay mad for a very long time”. Ms. Cosby shared that she did understands the importance of listening and getting all the information before making any comments. Ms. Cosby shared that she has tried of herself destructive behaviors. Ms. Cosby shared that she has not been violent or verbally abusive towards her family members due to the sessions and coping skills that QMHPA is teaching. Ms. Cosby acknowledged that she still feels embarrassed because her behaviors can be out of control and inappropriate. Ms. Cosby shared that she is willing to make behavioral changes as needed and be willing to take corrective measures to make the changes needed for her mental and physical health.
